Step 1: Check the map above to see if someone in your state has already submitted a proclamation request.
Step 2: If not, View Your State-by-State Information page to familiarize the specific requirements of your state.
Step 3: Complete your proclamation request by using the information and template provided.
Step 4: Let us know that you have submitted your proclamation request and when it is approved by using the form links below.
Step 5: Celebrate and Spread the Word! Share your efforts on social media using the hashtag #LLLDAM2025. Tag us using @AmputeeCoalition.
